Angeles National Forest Loop Fire (1966)

The  Loop Fire was a firefighting accident in the Angeles National Forest (see  10,000 Acres of San Gabriel Mountain Forest Gone Up In Smoke ) that claimed the lives of a dozen firefighters in November 1966.  The day after the disaster a comprehensive study was ordered and you get the results here . It's a quite impressive piece of work and it is still being used half a century later to train firefighters. But an investigation like this should have been ordered after every major forest fire, regardless of whether or not it involved fatalities . I have never done anything like this in my life. But I can imagine it would be really good.
