The  Loop Fire was a firefighting accident in the Angeles National Forest (see  10,000 Acres of San Gabriel Mountain Forest Gone Up In Smoke ) that claimed the lives of a dozen firefighters in November 1966.  The day after the disaster a comprehensive study was ordered and you get the results here .
